Bridging Healthcare and Social Services for People Living with Dementia and Their Caregivers

People living with dementia often have complex health and social support needs that can best be met by health care and community-based providers working together. This brief examines promising practices for bridging community-based organizations and health care systems in the providing comprehensive dementia care. The effort investigated the ways in which the two systems can work together to improve dementia care to answer research questions related to programs dedicated to supporting people living with dementia and caregivers through care management, training and education. The brief reflects on best practices to facilitate effective service delivery, as well as barriers to expanding the availability of dementia specific services in a coordinated manner.

This research was conducted under contract between HHS/ASPE’s Office of Behavioral Health, Disability, and Aging Policy (BHDAP) and the HHS Administration for Community Living (ACL) with Research Triangle Institute. Additional research in this area is available at the ASPE Aging & Disability page.
